NDIS Support Coordination for Brisbane participants

We are here to help Brisbane participants navigate the NDIS with our Support Coordination service.

What is Support Coordination?

The federal government define support coordination as “ a capacity building support to implement all supports in a participant’s plan, including informal, mainstream, community and funded supports.”

In simple terms, support coordination is an NDIS funded support, that exists to help participants get the most out of their NDIS plan.

What does a Support Coordinator do?

Your Brisbane based Support Coordinator will work with you to help you understand and get the most out of your plan. How your Support Coordinator will help: :

  • understand what your plan means and the flexibility within the plan
  • gain a greater understanding of the supports available in the plan
  • understand how the NDIS plan budget works
  • research and coordinate services that best meet your needs
  • identify and contact services and supports in your community that will assist you to reach your goals
  • develop agreements with service providers and obtain quotes for supports that meet your needs
  • coordinate services and supports
  • resolve points of conflict with providers
  • provide support, guidance and solutions in times of crisis
  • provide training to develop your capacity to confidently manage your plan over time
  • monitor NDIS plan outcomes and provide reports back to the NDIA
  • prepare for your child’s plan review
